Consent: By submitting your phone number, you consent to receive text messages and calls from Shockley Spine & Nerve Center regarding appointments, services, and promotions.
Opt-In/Opt-Out: You may opt out anytime by replying “STOP” to a text message or contacting us.
Message Frequency & Charges: Frequency may vary. Standard carrier message and data rates apply. We are not responsible for carrier delays or failures.
Digital Consent: Submitting your phone number via our website form constitutes your digital signature and acknowledgment of these terms.

8. SEVERABILITY

9. ENTIRE AGREEMENT

These Terms, together with our Privacy Policy, represent the entire agreement between you and Shockley Spine & Nerve Center, LLC.
